Nelson House is a modern and spacious 32 bed locked rehabilitation recovery service for men and women with mental illness. By having a focus on the development of skills for independence the unit provides a positive step for patients, many whose next step will be living in the community. It helps patients learn new ways of managing their conditions and symptoms with the aim of helping to prevent any future relapse. For patients with challenging behaviours, Nelson House helps them to learn skills to change their response to anxiety and distress. 

The role:

We are looking for a qualified Social Worker who will develop under supervision, Forensic Social Work skills.

The post holder will provide a Secure Mental Health Service which interfaces with The mental Health act 1983, Criminal Justice System, Secure Hospitals, Primary Care Trusts and the Community.  Responsible for the formulation of appropriate assessment, treatment and rehabilitation planning; within the context of a Multi-Disciplinary Team.  In collaboration with commission teams, users and develop risk assessed Aftercare Plans.

Duties:

  • To attend meetings as appropriate, ie. Clinical Directorates, Ward Rounds and deputise at CPA reviews upon request of the Social Workers.
  • To undertake supervision using agreed formats and at appropriate intervals.
  • To liaise with families and involved professionals to ensure necessary arrangements are made for family contacts.
  • To conduct as required one to one work with patients as consistent with the role and as identified through the CPA process.
  • To complete multi-disciplinary records of contact with patients.
  • To ensure hospital policies/procedures are maintained and complied with.
  • To ensure that health and safety and security policies are maintained and complied with via an audit system.

The Post holder will:

  • Respect confidentiality of both patients and colleagues.
  • Undertake other reasonable duties and training as required that are commensurate with the posts` level of responsibility.
  • Comply with the provisions of the Health & Safety at Work Act 1974 with regard to health and welfare of patients, staff and visitors.

Skills/Knowledge

  • Demonstrates an interest in Mental Health and Forensic Social Work.
  • Knowledge of CPA procedures.
  • An awareness of Equal Opportunities
