Grafton House is Community Rehabilitation for the continuation of neuro behavioural rehabilitation in a community setting to prepare the residents for living independently.

The role:

  • Promoting and providing high quality care
  • To be involved in developing, maintaining and reviewing care and treatment plans specific to individual identified care needs
  • To co-ordinate effective operational systems including staffing across both houses in line with skill mix and experience
  • To bridge effective communication channels between the Registered Manager and other team members when handing over and reviewing relevant information
  • To oversee and support the attendance of individual Annual Health checks and the completion of relevant documentation
  • To Create a homely and friendly environment for residents, staff and visitors
  • To follow all codes of conduct, standards and legislation, respecting the rights of others and promoting ethical practise
  • To adopt the local procedure for providing Key worker support including, providing evidence based practise
  • To undertake relevant training provided in the safe administration of medication and reordering of prescriptions supported by the Registered Manager
  • To act as a mentor and positive role model for newly appointed Community Support Assistants
  • To write and attend CPA and care plans
  • Timetables and meaningful week management
  • To promote independence and actively support residents to find meaningful occupation and activity
  • To support staff with neurobehavioral approaches and interventions
  • To be actively involved in all aspects of the running of the day to day needs of the service
  • To support the registered manager with audits and reports as required
  • To be part of the wider TDT process within the BIS service
  • To be on call in line with working shift pattern
