Avalon is a specialist residential home for adults with either a primary diagnosis mental health and/or an associated learning disability.  The home is ideally located for safe, structured and gradual reintegration into the community along with good transport link to Cardiff city centre.

Located near to central Cardiff, Avalon is a six bed specialist group home, providing short and long term rehabilitation for service users who have a learning disability with previous or current mental health issues.

The unit is led by an effective multidisciplinary team, experienced in rehabilitation, recovery and caring for service users with complex needs. We are committed to providing a safe environment in which service users can explore freedom of choice and expression and make decisions associated with everyday living.

We are looking to recruit personable, flexible and motivated Health Care Assistants to support our qualified staff with the delivery of care to our patients. You will be involved in the personal care and meeting the psychological needs of a diverse group of service users with varying degrees of mental and physical issues. You will be caring and attentive with the skills to build strong and trusting relationships with people. It is also important that you work well in a team as you will work alongside qualified  staff from other professions.

You will be required to:

  • Be Flexible and provide excellent standards of person centred support to the people who use our service, maintaining the highest levels of privacy and respect, enabling them to lead as independent lives as possible and realise their potential.
  • Ensure that the people who use our service have any personal needs or requirements met; including personal care, participation in activities and support with daily living tasks.
  • To support people who use our service in achieving their outcomes in the way that they choose.
  • Complete daily administration including updating person centred support & care plans, updating charts and completing accurate daily notes.
  • Through own professional behaviour, including adhering to policies and procedures, contributes to the maintenance of safety, security and confidentiality standards.
  • To undertake key working responsibility for individual service users, supporting the individual to plan goals and personal outcomes and ensure that these are comprehensively documented and reviewed.
